Doctor Lynn Aesthetics is committed to protecting and respecting your privacy. For your safety, we keep records about you, your health, and the care we plan or provide to you.
WHAT INFORMATION DO WE HOLD?
Name, address, DOB, phone number, e-mail, medical history, photographs, and limited payment card details (where card payment method is used)
WHAT DO WE DO WITH YOUR INFORMATION?
It is essential to obtain most of this information in order to provide good, safe clinical care.
Your information is only shared in the following situations:
- With pharmaceutical companies who provide me with products such as dermal fillers.
- During inspections by Healthcare Improvement Scotland, regulators may wish to view you records to ensure my practice is safe and of a high standard.
- In the event of a complaint or legal claim your information will be shared with both Doctor Lynn Aesthetics’s and your own legal representatives.
Anyone who receives information from us is also under a legal duty to keep your confidential safe and secure.
Photographs are sometimes shared on social media but only with the full consent of the client.
WHERE IS YOUR INFORMATION STORED?
Email and photographs are stored on a password protected computer. Photographs are stored on encrypted files.
Paper records and card payment receipts are stored in a locked filing cabinet in a locked room and can only be accessed by Dr Lynn Kelly. Doctor Lynn Aesthetics is Payment Card Industry compliant.
HOW CAN YOU ACCESS YOUR DATA?
You can access your data by putting in a request in writing or by emailing Dr Lynn Kelly. We will require proof of ID and address, and what information you require. We will fulfil your request with a month.
We will hold the personal data you provide for as long as you are a client of Doctor Lynn Aesthetics, and for 10 years after you cease to be a client of Doctor Lynn Aesthetics. This is to ensure you receive the correct care in the unlikely event of future complications.
In the unlikely event of a Data Breach, it is mandatory for Doctor Lynn Aesthetics to notify the Information Commissioners Office (ICO) within 72 hours of finding out it has occurred. Customers will also be notified without undue delay when a data breach has been identified.
CHANGES TO THIS PRIVACY NOTICE
We keep all our policies under regular review. This policy will be reviewed again 1 year from the date stated below.
CONCERNS ABOUT SHARING YOUR INFORMATION
If you have concerns about how we use or share your information or you do not wish us to share your information, please speak to Dr Lynn Kelly.
If you have a complaint about how your information is managed at the practice please speak to Dr Lynn Kelly in the first instance. If your complaint is not resolved then you can contact the Information Commissioner Office at www.ico.gov.uk
CHANGE OF DETAILS
It is important that you tell us if any of your details such as your name, address or telephone number have changed. You have a responsibility to inform us of any changes so our records are kept accurate and up to date for you.
